Fortunately, many identity theft victims have incorrectly used form 3949-A to notify the IRS of their problem. This form is incorrect because it is supposed to be used to report fraudulent tax activity. Unfortunately, some of these folks were advised to use that form at the IRS Assistance Centers. The IRS says their correcting that problem.

Unfortunately, the Internal Revenue Service has admitted that they mistakenly destroyed about $3,000 forms filed by identity theft victims.
